Background: Cervical cancer poses a significant global health challenge, particularly affecting women in developing regions. In India, it stands as the second most prevalent cancer among women, contributing substantially to the overall cancer burden. This study focuses on District Shimla, Himachal Pradesh, exploring the awareness and knowledge levels regarding cervical cancer and the Human Papillomavirus (HPV) vaccine among women of reproductive age. Material and Methods: Employing a descriptive cross-sectional survey design, the study spanned from August 2023 to October 2023. The target population comprised women residing in District Shimla for at least 12 months. A sample size of 400 participants was determined, and a Google Form questionnaire covering socio-demographic details and 20 structured knowledge-based questions on cervical cancer and the HPV vaccine was utilized. Data collection occurred through online platforms, and analysis was conducted using Epi Info V7 software. Results: Among the 400 respondents, 61% hailed from rural and 39% from urban areas. Knowledge levels varied, with 30.25% demonstrating very good, 35.5% good, 21.5% fair, and 12.75% poor knowledge regarding cervical cancer and the HPV vaccine. Key gaps included a nuanced understanding of HPV's role in cervical cancer and misconceptions about vaccine-related risks. Variations in preventive measure awareness and HPV vaccine understanding were observed. Conclusion: While a commendable baseline awareness about cervical cancer exists in District Shimla, persistent gaps warrant targeted educational interventions. The study emphasizes the need for context-specific awareness campaigns tailored to the unique demographics of the region. These findings serve as a foundation for informed public health policies, aiming to empower women with the knowledge necessary for proactive health decisions.
Cervical cancer is a global health challenge of paramount concern, with particular resonance in developing regions. In India, cervical cancer ranks as the second most prevalent cancer among women, constituting a substantial share of the overall cancer burden. The state of Himachal Pradesh, characterized by its unique demographics and geography, is not exempt from this public health challenge. As we explore the intricacies of women's health in this region, it becomes increasingly crucial to fathom the existing awareness levels and knowledge regarding cervical cancer and the Human Papillomavirus (HPV) vaccine among women of reproductive age in District Shimla [13].
The etiology of cervical cancer is primarily linked to persistent infection with high-risk types of HPV, a sexually transmitted virus. The advent of the HPV vaccine represents a powerful preventive strategy, holding the potential to significantly reduce the incidence of cervical cancer. However, the effectiveness of this intervention hinges on widespread awareness and understanding among the target population, particularly women of reproductive age [4-5].
While global efforts have sought to address cervical cancer through vaccination programs, screening initiatives, and awareness campaigns, the effectiveness of these measures can vary widely based on regional disparities. Therefore, a localized understanding of the awareness landscape and knowledge gaps is crucial for tailoring interventions to the specific needs of the population [6-7].
This study aims to assess the awareness and knowledge levels regarding cervical cancer and the HPV vaccine among women in District Shimla, Himachal Pradesh. By delving into the nuanced dynamics of awareness, we aim to identify potential barriers to HPV vaccine uptake and gaps in knowledge. This information is not only instrumental for developing targeted interventions but also serves as a foundation for informed policymaking to enhance awareness and promote proactive health-seeking behaviors among women in the region.
Objectives of the Study
To evaluate the awareness and knowledge regarding Cervical Cancer and Human Papilloma Virus (HPV) Vaccine among women of reproductive age group in District Shimla ,Himachal Pradesh

Study Area: District Shimla, Himachal Pradesh
Study Duration: Between August 2023 to October 2023
Study Population: All women of reproductive age group who were staying in the District Shimla, Himachal Pradesh for 12 months or more
Sample Size: 400 women of reproductive age group assuming 50% have adequate knowledge regarding Cervical Cancer and Human Papilloma Virus ( HPV) Vaccine , 5% absolute error, 95% confidence level, and 5% non response rate
Study Tool: A google form questionnaire consisting of questions regarding socio-demography and Cervical Cancer and Human Papilloma Virus (HPV) Vaccine was created. The questionnaire was initially pre-tested on a small number of women to identify any difficulty in understanding by the respondents
Description of Tool
Demographic Data Survey Instrument: The demographic form elicited information on participants’ background: age, marital status, religion, employment, education and many more
Questionnaire: The questionnaire contains 20 structured questions regarding knowledge about Cervical Cancer and Human Papilloma Virus (HPV) Vaccine. One mark was given for each correct answer and zero for incorrect answer. The maximum score was 20 and minimum score was zero in each category. Scoring was done on the basis of marks as >80%(16-20) = very good, 60-79% (12-15) = Good, 41-59% ( 8-11) = Fair, <40% (< 8) = poor
Validity of Tool: By the experts in this field
Data Collection: Data was collected under the guidance of supervisors. The google form questionnaire was circulated via online modes like e-mail and social media platforms like Whatsapp groups, Facebook, Instagram and Linkedin in both rural and urban area of District Shimla, Himachal Pradesh till the 400 responses were collected.
Data Analysis: Data was collected and entered in Microsoft excel spread sheet, cleaned for errors and analyzed with Epi Info V7 Software with appropriate statistical test in terms of frequencies and percentage
Ethical Considerations: Participants confidentiality and anonymity was maintained
The goal of the current study was to assess the awareness aboutCervical Cancer and Human Papilloma Virus (HPV) Vaccine among women of reproductive age group in District Shimla Himachal Pradesh through a non-experimental descriptive survey
A total of 400 respondents took part in the study, with 244 (61%) coming from rural areas and 156 (39%) from urban areas (Table 1).

The comprehensive evaluation of cervical cancer awareness and HPV vaccine knowledge among women of reproductive age in District Shimla, Himachal Pradesh, provides valuable insights for public health interventions. Our findings, in conjunction with existing literature, contribute to the nuanced understanding of awareness gaps and highlight the need for context-specific initiatives.
Our study revealed that 73.25% of participants were aware of what cervical cancer is and which part of the body it affects, indicating a foundational knowledge level. However, only 31.25% understood the role of Human Papillomavirus (HPV) in cervical cancer development. This aligns with studies in diverse settings, emphasizing the necessity of targeted educational efforts to enhance understanding of the HPV-cervical cancer link [6-7].
Concerning preventive measures, 60.25% demonstrated awareness of common ways to reduce the risk of HPV infection. While this indicates a reasonable level of knowledge, continuous efforts are crucial to ensure a comprehensive understanding among the entire population [8]. Addressing this gap is paramount, as preventive measures play a crucial role in reducing the burden of cervical cancer.
The study also unveiled variations in awareness of the HPV vaccine, with 60% correctly identifying the recommended age group and the required number of doses. However, only 49.25% accurately understood the duration of vaccine protection. Misconceptions and myths were reported by 49.25% of participants, emphasizing the importance of addressing misinformation through targeted educational initiatives [9]. Comprehensive educational programs are essential to dispel myths and foster informed decision-making regarding vaccination.
Comparison with existing literature, including studies in India by Sreedevi et al. [2]and the National Cancer Registry Programme [3], underscores the persistent challenges in cervical cancer prevention. Our findings align with these studies, reinforcing the need for culturally sensitive and locally tailored awareness campaigns [10]. Cultural and societal beliefs continue to influence health-seeking behaviors, necessitating a multifaceted approach for effective intervention [11].
In conclusion, this study offers a comprehensive assessment of cervical cancer awareness and HPV vaccine knowledge among women in District Shimla, Himachal Pradesh. While a commendable baseline awareness is evident, particularly regarding the general understanding of cervical cancer, persistent gaps exist, emphasizing the need for targeted educational interventions. Our findings align with global efforts to address cervical cancer and underscore the importance of context-specific strategies. The study contributes valuable insights to the existing literature, and the identified knowledge gaps serve as a foundation for the development of culturally sensitive awareness campaigns tailored to the unique demographic and geographic characteristics of the region. As we navigate the challenges in cervical cancer prevention, this research serves as a catalyst for informed public health policies and interventions aimed at empowering women in District Shimla with the knowledge and resources to make informed decisions about their health, ultimately contributing to the reduction of cervical cancer burden in the region and beyond.
