Epworth Sleepiness Scale (ESS) in Diagnosis of OSA

Abstract

Aims and Objectives: To study the role of ESS in diagnosis of OSA. Material and Methods: Sample size: 50. patients visiting Pulmonary Medicine OPD. Inclusion Criteria: Patients age 20 years and above. Patients with BMI more than 25. Patients consenting for study. Patients on regular follow-up in OPD. Exclusive Criteria: Patients not consenting for study. Patients with active tuberculosis. Patients on anti-psychotic medications. Patients with GCS less than 12.

INTRODUCTION

OSA is a common sleep disorder among the patients visiting Pulmonary Medicine OPD. It is especially more common in obese patients. Sedentary lifestyle with eating fast-food is the major cause of developing obesity. If we are able to predict the risk factors and diagnose the high risk patients early, we can significantly reduce the mortality and morbidity associated with the disease [1-3]. ESS score is a non-invasive technique based on question arises to be answered by the patients. We segregate the high risk patients based on their ESS score and post them for sleep study. Those diagnosed with OSA are initiated on appropriate interventions including CPAP therapy based on the disease severity. We aim to validate the ESS score and correlate it with the sleep study [4].

 

Aims and Objectives

To study the role of ESS in diagnosis of OSA.

MATERIALS AND METHODS

Sample size:50

 

Inclusion Criteria

 

  • Patients age 20 years and above

  • Patients with BMI more than 25

  • Patients consenting for study

  • Patients on regular follow-up in OPD

 

Exclusive Criteria

 

  • Patients not consenting for study

  • Patients with active tuberculosis

  • Patients on anti-psychotic medications

  • Patients with GCS less than 12               

 

Methodology

We included the patients visiting our Pulmonary Medicine OPD for our study. We took the ESS score of the patients meeting the inclusion criteria and posted these patients for sleep study [5].

RESULTS
  • We had male predominant population with 70% males  and 30% female

  • Age group of the study population was 48+-5

  • ESS score of our study population was 12

  • 98% were diagnosed with OSA after sleep study

  • 1% patient left follow-up during the study period

  • 20% patients had mild OSA and were advised lifestyle modification

  • 46% patients required CPAP therapy

CONCLUSION

ESS score is a promising non-invasive score useful in diagnosis of OSA.
