Background: Regional anesthesia techniques, including epidurals, spinals and nerve blocks, play a pivotal role in modern perioperative pain management. Their application offers advantages such as targeted pain relief and reduced opioid consumption. This study assesses the knowledge and understanding of these techniques among Resident Anesthetists in Himachal Pradesh, India. Materials and Methods: A cross-sectional survey was conducted among Junior and Senior Residents of Anesthesia working in Medical Colleges in Himachal Pradesh. A robust sample size of 100 was determined. A questionnaire containing 20 structured knowledge-related questions was utilized. Data were collected through online modes and statistical analysis was performed using Epi Info V7 Software. Results: The participants exhibited commendable knowledge across various aspects of regional anesthesia. Notably, 88% correctly understood the primary goal of regional anesthesia and its distinctions from general anesthesia. Furthermore, participants demonstrated proficiency in differentiating between epidural and spinal anesthesia techniques (81% correct responses) and explaining their indications (76% correct responses). The participants also displayed strong knowledge regarding anatomical landmarks for lumbar epidural placement (72% correct responses) and were aware of potential complications and contraindications of epidural anesthesia (71% correct responses). Moreover, they exhibited understanding of nerve blocks, including brachial plexus nerve blocks (67% correct responses), axillary nerve blocks (62% correct responses) and sciatic nerve blocks (61% correct responses). The importance of monitoring during regional anesthesia procedures was recognized (74% correct responses). In terms of patient consent and documentation, 81% of participants provided correct descriptions. Conclusion: The study reveals that Resident Anesthetists in Himachal Pradesh possess commendable knowledge of regional anesthesia techniques and their applications. Their proficiency in understanding the nuances of these techniques bodes well for patient care. Continuous education and training in regional anesthesia can further enhance their competence, ultimately contributing to improved healthcare standards in the region.
Regional anesthesia techniques, including epidurals, spinals and nerve blocks, have emerged as invaluable tools in modern anesthesiology, revolutionizing the landscape of perioperative pain management. These techniques offer numerous advantages, such as targeted pain relief, reduced opioid consumption and improved postoperative outcomes. In the breathtaking state of Himachal Pradesh, where the tranquil beauty of the Himalayas coexists with the pursuit of healthcare excellence, Resident Anesthetists play a pivotal role in providing safe and effective regional anesthesia to patients undergoing various surgical procedures [1,2].
The field of regional anesthesia is dynamic and ever-evolving, encompassing a diverse range of techniques and applications. Epidurals, spinals and nerve blocks stand out as cornerstones of regional anesthesia, each with its unique indications and advantages. Epidural anesthesia involves the injection of local anesthetics and opioids into the epidural space, providing effective pain relief for surgeries, labor and chronic pain conditions. Spinal anesthesia, on the other hand, involves the injection of local anesthetics directly into the cerebrospinal fluid, resulting in rapid and profound anesthesia of the lower body. Nerve blocks entail the precise deposition of anesthetic agents near peripheral nerves, offering targeted pain relief for specific regions of the body [3-5].
The proficiency and knowledge of Resident Anesthetists in Himachal Pradesh regarding these regional anesthesia techniques are of paramount importance. The effective utilization of these techniques demands a deep understanding of anatomy, pharmacology and clinical applications. Moreover, staying abreast of the latest advancements in the field is essential to provide patients with the best possible care.
This study embarks on a journey to explore the realm of regional anesthesia among Resident Anesthetists in Himachal Pradesh. The primary aim is to assess their levels of awareness and knowledge regarding epidurals, spinals and nerve blocks and their applications. By doing so, we seek to gain insights into the readiness of these anesthesia professionals to harness the potential of regional anesthesia techniques to improve patient care.
Objectives of the Study
The primary objective of this study is to gauge the levels of awareness and knowledge about various regional anesthesia techniques like epidurals, spinals and nerve blocks and their applications among the Residents of Anesthesia in Himachal Pradesh.
Research Approach: Descriptive
Research Design: Cross-sectional survey design
Study Area: Himachal Pradesh
Study Duration: Between April 2023 to June 2023
Study Population: The study's target population encompassed all Junior and Senior Residents of Anesthesia working in Medical Colleges in Himachal Pradesh
Sample Size: A robust sample size of 100 was determined using a 95% confidence level, an estimated knowledge level of 50% regarding various regional anesthesia techniques, a precise 10% absolute error margin and a conservative 5% non-response rate
Study Tool:A google form questionnaire consisting of questions regarding socio-demography and knowledge regarding various regional anesthesia techniques was created. The questionnaire was initially pre-tested on a small number of participants to identify any difficulty in understanding by the respondents
Description of Tool
Demographic Data Survey Instrument: The demographic form elicited information on participants’ background: age, gender, marital status, religion etc
Questionnaire: The questionnaire contains 20 structured knowledge related questions regarding various regional anesthesia techniques. One mark was given for each correct answer and zero for incorrect answer. The maximum score was 20 and minimum score was zero. Scoring was done on the basis of marks as >80% (16-20) = very good, 60-79% (12-15) = Good, 41-59% (8-11) = Fair, <40% (<8) = poor
Validity of Tool: By the experts in this field
Data Collection: Data was collected under the guidance of supervisors. The google form questionnaire was circulated among all the Junior and Senior Residents of Anesthesia working in Medical Colleges in Himachal Pradesh for responses using online modes like e-mail and social media platforms like Whatsapp groups, Facebook, Instagram and Linkedin till the 100 responses were collected
Data Analysis: Data was collected and entered in Microsoft excel spread sheet, cleaned for errors and analyzed with Epi Info V7 Software with appropriate statistical test in terms of frequencies and percentage
Ethical Considerations: Participants confidentiality and anonymity was maintained
The primary objective of this study was to gauge the understanding of various regional anesthesia techniques like epidurals, spinals and nerve blocks and their applications among the Residents of Anesthesia in Himachal Pradesh. A total of 100 participants took part in the study (Table 1).

In the present study 24 participants had very good knowledge (16-20 marks) towards various regional anesthesia techniques, 35 had good knowledge (12-15 marks), 22 had fair knowledge (8-11 marks) and 19 having poor knowledge (<8 marks) (Table 2).
Regional anesthesia techniques, including epidurals, spinals and nerve blocks, have transformed the landscape of perioperative pain management, offering numerous advantages. This study aimed to assess the knowledge and understanding of these techniques among Resident Anesthetists in Himachal Pradesh and their applications in clinical practice. The results provide valuable insights into their proficiency in utilizing regional anesthesia to enhance patient care.
The study findings demonstrate a commendable grasp of the primary goal of regional anesthesia and its differences from general anesthesia. Resident Anesthetists recognize that regional anesthesia provides targeted pain relief without inducing complete unconsciousness, aligning with its fundamental purpose. Participants exhibited good knowledge regarding the key differences between epidural and spinal anesthesia techniques. Understanding these differences is essential for making informed decisions in clinical practice, considering factors like surgical requirements and patient preferences [2,3].
Moreover, the ability to explain the indications for choosing between epidural and spinal anesthesia highlights the participants' clinical acumen. This knowledge is crucial for tailoring anesthesia plans to individual patient needs. The study revealed that participants possess a solid understanding of the essential anatomical landmarks and patient positioning for lumbar epidural placement. Proficiency in these aspects is crucial for the safe and effective administration of epidural anesthesia. Additionally, participants demonstrated awareness of potential complications and contraindications of epidural anesthesia. Recognizing these factors is pivotal for risk mitigation and patient safety. The knowledge displayed regarding Combined Spinal-Epidural (CSE) anesthesia, including its advantages and disadvantages, is noteworthy. CSE anesthesia offers flexibility and rapid onset and understanding when to use it is essential for optimizing patient care [4,5].
Participants showcased familiarity with the technique for performing a subarachnoid block (spinal anesthesia) and its primary clinical applications. Spinal anesthesia is a cornerstone technique in regional anesthesia and this knowledge reflects their preparedness for its administration. The ability to discuss the choice between single-shot spinal blocks and continuous spinal anesthesia indicates participants' awareness of various approaches to spinal anesthesia, catering to diverse clinical scenarios [2,3].
Participants exhibited proficiency in explaining the principles of peripheral nerve blocks and their role in regional anesthesia. Nerve blocks offer targeted pain relief and enhance recovery and their understanding is crucial for comprehensive patient care. Knowledge of specific nerve blocks, such as brachial plexus and axillary nerve blocks and their common surgical applications is vital. This demonstrates participants' readiness to provide specialized anesthesia for various surgical procedures [3,4].
The understanding of regional anesthesia's role in managing postoperative pain and improving recovery aligns with contemporary pain management strategies. Participants displayed awareness of potential complications and risks associated with regional anesthesia techniques. Additionally, recognizing the importance of monitoring during regional anesthesia procedures highlights their commitment to patient safety. The study revealed that participants recognize the significance of ultrasound-guided techniques in improving the precision of nerve blocks. This aligns with current trends in regional anesthesia, emphasizing enhanced visualization for accurate block placement. Understanding the concepts of continuous catheter-based regional anesthesia and its advantages underscores the participants' awareness of innovations in the field, providing extended pain relief for patients Recognizing the importance of appropriate patient consent and documentation for regional anesthesia ensures ethical practice and patient-centered care [4,5].
The findings of this study reveal that Resident Anesthetists in Himachal Pradesh possess commendable knowledge and awareness of various regional anesthesia techniques and their applications. Their proficiency in understanding the nuances of these techniques bodes well for providing safe and effective anesthesia care to the region's patients. Continuous education and training in regional anesthesia can further enhance their competence, ultimately contributing to improved healthcare standards in this picturesque region.
